  • Live vaccines: MMR, Varicella, Rotavirus — avoid in immunocompromised patients.
  • Tdap booster: At age 11–12 years.
    • DTaP - Used for infants and young children.
    • Tdap - Used as a booster for older children
The uppercase letter refers to a standard dose of antigen. Lowercase letters are used to indicate a reduced dose of the antigen. In the DTaP vaccine, there is a standard dose of Diphtheria, Tetanus, and Pertussis antigens. In the Tdap, there is a standard dose of Diphtheria but a reduced dose of Tetanus and Pertussis antigens. The "a" stands for acellular indicating that it contains parts of the pertussis bacteria.
  • HPV vaccine: Recommended at 11–12 years, can be started as early as 9.

At birth:

  • Hepatitis B (Hep B)

At 2, 4, 6 months (3 doses total):

  • Hep B
  • Rotavirus (oral; live-attenuated)
  • DTaP (Diphtheria, Tetanus, Pertussis)
  • Haemophilus influenzae type B (Hib)
  • Pneumococcal conjugate (PCV13)
  • Polio (IPV) (Salk; IM killed)

6 Months and Annually

  • Influenza (IIV – inactivated; start at 6 months)
    • 2 doses 1 month apart the first time it's given (if <9 yrs old and naive)
    • Then yearly

12–15 Months

  • Hib (final dose)
  • PCV13 (final dose)
  • MMR (Measles, Mumps, Rubella – live)
  • Varicella (live) (1st dose)
  • Hepatitis A (2-dose series, 6 months apart)
  • TDaP (4th dose)

At 4-6 years:

  • DTaP(5th dose)
  • Polio (4th dose)
  • MMRV (2nd dose)
  • Varicella (2nd dose)

At 11-12 years:

  • Tdap (booster)
  • HPV (1st dose)
  • Meningococcal (MenACWY) (1st dose)

At 16-18:

  • Meningococcal (MenACWY) (booster)

Age 9-45:

  • HPV (2 doses if first given age 9-14; 3 doses if first given age 15+)

Age 50+:

  • Herpes zoster (VZV) - Shingles vaccine
